PrEP FORMS NOT AVAILABLE

Viewed 14

We have noted that for a majority of clients that are eligible and willing to take PrEP, the PrEP Enrol button in Care Panel is not available and consequently there are no PrEP forms to fill. (Bomu Changamwe on v19.2.0_13062025).

PrEP form is validated to appear only if the patient meets the eligibility creteria outlined below:

• HIV status (Client must be HIV- Negative)
• Age – (Client must be aged above 14 years)
• Weight- (Clients current weight must be above 30kgs)
• Willingness to start PrEP – (Client must have consented to PrEP and this documented in the Risk assessment Tool RAST).
• Creatinine test result – (Where a creatinine test has been done, the results must be > or = 50mL/Min.)

Check to be sure your patient is eligibile based on the above.
